Our mission

Our mission is to provide high-quality, community-based care in a home-like setting for patients who are transitioning between acute/hospital care and home care.

We’re in it together

At Sunny Sands Manor, we believe recovery is a team effort between the resident, their family, and the people caring for them every day. Nobody gets there alone.

We work to keep the house feeling like a house. Meals are cooked in the kitchen, the living room is for living, and the medical equipment stays where it is needed and out of the way where it is not.

Our staff are experienced, licensed, and chosen for how they treat people when nobody is watching. Respect, patience, and plain honesty are the standard here, and anything less does not last.

Together, we get residents home, or as close to home as their health allows.

Frequently Asked Questions

1. What is a Congregate Living Health Facility (CLHF)?
A CLHF is a California licensed residential health facility, usually a regular house with a small number of beds, that provides 24 hour skilled nursing for people whose medical needs are too complex for home care but who do not need a full hospital. It is licensed and inspected by the California Department of Public Health.
2. What type of care can a CLHF provide?
Skilled nursing, wound care, ventilator and tracheostomy care, tube feeding, IV medications, physical and occupational therapy, and daily living support, depending on the facility's license category. Ask us about the specific services we are licensed for.
3. Who is eligible to receive care here?
Adults who need ongoing skilled nursing and whose physician confirms a CLHF is the right level of care. Most residents come to us after a hospital or rehab stay. We review each referral individually and will tell you honestly if we are not the right fit.
4. What makes Sunny Sands Manor different?
Small by design: a real house, a small number of residents, and the same nurses most days. Ask us about our staff ratio, physician visit schedule, and family visiting policy.
5. How do I find out if someone is eligible for admission?
Call or send a message through the contact page. We will ask a few questions about the person's condition, insurance, and current location, then arrange an assessment and a tour if it looks like a fit.
